Care Manager / Marketer (Medicaid, MCOs)

The Care Manager is responsible for coordinating, implementing, and supervising the delivery of services to agency clients in alignment with the company’s mission and standards of care. This position ensures compliance with regulatory and agency requirements for service provision and maintains client records.

MINIMUM QUALIFICATIONS:

  • Two years of experience in home care, home health, or related field.
  • A reliable means of transportation with current insurance and a valid driver’s license.
  • Excellent written and verbal communication skills; ability to interact effectively with clients, families, care providers, and professionals.
  • Impeccable follow-up, attention to detail, initiative, problem-solving, and sound judgment.
  • Ability to supervise, assess, evaluate care providers, and advocate for clients and families.
  • Passionate about helping others live with dignity.
  • Professional business image.
  • Pass a criminal background check.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Complete Client Assessment for each new client; provide signed agreements to client or representative.
  • Develop and implement care plans, including experience with Medicaid clients dealing with MCOs, United Healthcare, Molina, and Community First Health Plan, covering:
    • Client’s functional limitations
    • Nutritional needs and allergies
    • Home medical equipment and supplies
    • Type and schedule of services
    • Non-medical tasks
  • Collect fees at assessment or first day of service.
  • Introduce caregivers, review care plan, and set service expectations; ensure accessible in-home care plan binder.
  • Supervise client care to ensure caregivers follow the care plan, proper infection control, recognize changes, and possess necessary skills; ensure supplies are available and services are within license scope.
  • Communicate regularly with clients and representatives to ensure satisfaction.
  • Conduct scheduled and unscheduled home visits; perform reassessments as needed.
  • Coordinate with other community services involved in client care.
  • Serve as a resource for education, empowerment, and referrals.
  • Investigate client and employee issues; document supervision and maintain records.
  • Provide staff training and education; report pertinent information for service delivery and scheduling.
  • Assist in developing and revising policies and participate in caregiver evaluations.

Compensation: $15.00 - $18.00 per hour
