Preceptor Home Health & Hospice - Part-Time Music Therapist

We are seeking a Part-Time Music Therapist with geriatric or hospice experience for our Milwaukee and Waukesha County territories. This position involves working approximately 10-20 hours per week.

Responsibilities include:

1. Develop individualized treatment plans based on assessments.
2. Provide music therapy to individual patients using instrumental, vocal, or recorded music to help reduce physical symptoms such as pain, agitation, and shortness of breath.
3. Complete documentation after each visit.
4. Adjust care plans as needed.
5. Participate in team meetings.

Job Qualifications:

1. Bachelor's degree in music therapy from an accredited institution approved by the American Music Therapy Association.
2. Must be board certified (MT-BC).
3. Licensed and insured driver with the ability to travel to client homes; mileage reimbursement provided.
4. Excellent verbal and written communication skills, along with interpersonal and organizational skills.
5. Ability to work independently.
6. Previous hospice experience preferred.
