Monday-Friday, 8am-3:30pm or until the last consumer leaves; occasional staff meetings outside these hours.

Hourly:

$25.35/hour - $30.81/hour

Job Summary:

The Media Lab is a progressive film and media program that teaches valuable life and vocational skills to adults with disabilities, through the art of film and broadcast television. We are seeking a skilled and compassionate Case Manager to join our team. The Case Manager is responsible for a caseload of approximately 30-45 individuals. They will work with the Media Specialists and other studio staff to help develop goals and provide support to students on caseload.

Responsibilities:

  • ensure on-going communication with planning team,
  • maintain a caseload of 30-45 clients and be responsible for scheduling and developing the ISP and semi-annual reviews, and annual reviews,
  • complete necessary paperwork, reports, etc. on a timely basis,
  • confer with the planning team for 30-day review, semi-annual review, annual review and any IDT meetings,
  • evaluate consumer progress and plan as identified in the Individual Service Plan (ISP) and Individual Program Plan (IPP) on an ongoing basis,
  • maintain client case file and notify appropriate parties for expiring documents (i.e. medical assessments, expired CDER, etc.),
  • complete special incident reports on a timely basis and inform representatives of other programs/agencies as appropriate,
  • participate in and assist clients in fire and earthquake drills,
  • assure consumers' rights and dignity are maintained in the provision of services.
  • provide guidance and mentorship to students on a daily basis

Qualifications:

  • Bachelor’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development or a related field from an accredited college or university
  • 2 years of experience in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
  • Must be a strong writer and must demonstrate excellent verbal communication skills.
  • Previous experience working with the Regional Center in the provision of person-centered practices for people with disabilities is plus.
  • Computer proficiency in Google Workspace, Zoom, Dropbox, Microsoft Office Suite – Word, Excel, PowerPoint.
  • Proficiency in Adobe Premiere Pro and basic video production knowledge is a plus, although not required.
  • Must pass background investigation that includes FBI/DOJ fingerprint check.
