Overview Aventura At West Park is actively seeking an amazing external/internal admissions director for our Skilled Nursing Facility. Are you an experienced admissions director? Requirements must have 2 years of SNF experience as an Admissions Coordinator or Director.

Responsibilities

Coordinating the resident referral and approval process efficiently

Assist the marketing team in establishing & maintaining relationships with local hospitals, physicians, and community organizations to generate consistent admissions to the facility

Follow and utilize the admissions process including policy, procedures, assessments, and paperwork requirements

Support the facility's Public Relations to generate a positive image & encourage word-of-mouth referrals

Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes

Communicate special needs of new admissions to staff to ensure a smooth transition

Provide appointments for facility presentations and on-site tours

Salary ADMISSIONS COORDINATOR SALARY: 55K-70K (depending on experience)

Additional Qualifications

2 yrs Prior experience as post-acute and long-term care Admissions Coordinator

Understanding of clinical and financial review process of nursing home admissions

Ability to work independently and with a team

Experience in community relations preferred

Be self-driven with excellent internal sales/marketing skills

Ability to implement the process to achieve desired outcomes

Possess communication, time management, and organizational skills

Solid computer skills; working knowledge of MS Office, PCC, CRM, and Excel
