Clinic offers : Inhouse Consulting Specialist : Cardiology, Podiatry and more Holistic Health Services : Acupuncture, Tai Chi, Yoga and Nutrition Door to Door Transportation for our patients (Uber / Lyft) Responsibilities Salary : Base $280,000-$300,000 Hours :

Monday – Friday work schedule; Clinic Hours 7 : 30a – 5 : 00p (last patient is scheduled at 4 : 15 Work / Life Balance Small Patient Panel – 60-90 patients’ max Manage PCP’s and NP’s Patient Population :

Seniors with 3-5 chronic diseases Value Based Care Qualifications Board Certified / Board Eligible in : FM, IM or Geriatric Medicine MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty. At least 1 year of clinical experience in a geriatric or family practice setting. Leadership experience preference Skills :
