is looking for a dynamic
Patient Navigator
to join our growing team. This is a
field-based role
focused on building strong relationships and promoting our exceptional hospice services in the community.
About the Role
As a Patient Navigator, you'll spend the majority of your time
out in the field , meeting with providers, educating community partners, and establishing referral relationships. This is
not a desk job -we're looking for someone who thrives on meaningful interactions, strategic outreach, and making connections that help patients receive the care they need.
What You'll Do
Promote hospice services to physicians, hospitals, discharge planners, and community groups Conduct regular outreach and in-person visits to maintain and grow referral networks Educate providers and partners on hospice care and eligibility Represent Advanced Hospice of Ohio at community events, health fairs, and networking opportunities Track leads, referrals, and activity in the CRM system Collaborate with clinical teams to align messaging and ensure quality patient transitions Requirements
Previous marketing experience
(required) 1+ year of healthcare marketing or field outreach experience
(preferred) Knowledge of hospice or healthcare services is a plus Valid driver's license
and
reliable transportation
