Key Job Responsibilities
A music therapist is trained and skilled in music therapy for therapeutic benefits. Music therapy identifies client responses to different styles of music, such as improvising or listening, or elements of music, such as tempo and harmony. This position supports The Care Team Hospice's commitment to service excellence. The Music Therapist provides an initial assessment for referred hospice patients, determines therapeutic goals, and works with individuals, family members, and staff to meet these goals using a systematic application of music interventions in the patients' homes or alternate community settings.
- Assess and provide individualized music therapy consultations and services to the patient and/or family.
- Develop an individualized music therapy plan of care.
- Use creative therapeutic activities to improve or maintain mental and physical function and quality of life.
- Implement the music therapy interventions by the interdisciplinary plan of care.
- Direct and implement legacy projects for patients/families as identified in the plan of care.
- Direct other care team members in the use of music to achieve individualized care plan goals.
- Document all services provided to hospice patients and families in the clinical record.
- Terminate therapy services when appropriate.
- Support the business development activities of the organization.
- Provide in-services to hospitals, nursing homes, and other facilities as requested and determined appropriate.
- Support bereavement activities as needed and appropriate.
- Provide bereavement counseling to family members as identified.
- Participate in and/or facilitate Hospice memorial services.
- Facilitate grief support groups as identified by the IDG.
- Complete documentation timely within 48 hours of visit.
- Serve as the counselor at IDG meetings, participate in on-call rotation, and assist with sign-ups and consents for newly referred patients if needed.
- Educate the hospice staff about the role of music therapy and the use of music therapy intervention with hospice patients and staff.
Qualifications
- Maintains the agency's mission, philosophy, and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
Regulatory Requirements
- Licensed or registered in the State of IN
- Must pass a criminal background check & MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state driver\'s license.
- Proof of current automobile insurance.
- TB testing per agency policy; 1- or 2-step TB skin test within 12 months of hire and annual TB symptom screening thereafter.
Industry Experience
- Hospice and Palliative Care certified Music Therapist preferred.
Education
- Bachelor\'s Degree in Music with a concentration in Music Therapy and must meet educational requirements to achieve certification from the Certification Board for Music Therapists. Bachelor\'s Degree in Music Therapy or a Master\'s Degree in Music Therapy is preferred. Previous experience with terminally ill patients/families is highly desirable.
