THE ORGANIZATION

A private nonprofit organization founded in 1973, Philadelphia Corporation for Aging (PCA) is a large agency that is part of the Area Agency on Aging (AAA) nationwide network. Founded as a result of the federal Older Americans Act, the AAAs are mandated to "facilitate the area-wide development and implementation of a comprehensive, coordinated system for providing long-term care in home and community-based settings, in a manner responsive to the needs and preferences of older individuals and their family caregivers." PCA carries out this mission through advocacy, stewardship of public funding, planning, development and administration of programs, and service coordination.

Description of Role: Under the oversight of the Assistant Director for Health and Nutrition Services, and supervised directly by the Health Promotion Nurse Supervisor in Community Engagement, and working with senior community center and other community staff, is responsible for planning, developing, implementing, delivering, evaluating, and monitoring senior center and other community-based Health Promotion Programs, including National Council on Aging (NCOA)-approved evidence-based programs.

Requirements

Required Experience and Education:

Education:
  • Master's Degree in Public Health, or other health, nutrition or exercise-related field.
OR
  • Bachelor's Degree in Public Health, nutrition, exercise, or other health-related field, plus six years of experience as indicated below:
Experience:
  • Minimum of 3 years' community-based work with older individuals, families, and groups in: program planning, delivery and management, health assessment, teaching.
  • Knowledge of current health and aging literature, sensitive to the special and cultural needs of older adults, able to assess individual and community needs for health promotion, and able to plan, coordinate, implement, and evaluate health promotion programs.
  • Experience in initiating community and academic contacts and leading discussion groups, planning workshops, and/or conducting training.
  • Familiarity with NCOA-approved evidence-based programs addressing topics such as chronic disease, diabetes, pain self-management, falls prevention and exercise preferred.
  • Experience as a Master trainer, certified diabetes educator, or in delivering evidence-based programs a plus.
  • A valid driver's license is required.

a. Researches, identifies and recommends evidence-based health promotion programs for use in senior centers and other community-based sites.

b. As a Master Trainer, recruits and trains lay leaders and senior center staff to conduct evidence-based programs, as applicable. Schedules workshops, monitors leaders, ensures fidelity of programs. Maintain certification and retention of peer/lay leaders.

c. Conducts marketing and publicity campaigns to recruit peer/lay leaders to facilitate evidence-based program workshops, workshop attendees and host sites for workshops.

d. Ensures all required data collection for evidence-based programs is completed and entered in appropriate databases in a timely manner.

e. Maintains sufficient inventory of required supplies for programs (books, charts, CDs, etc.)

f. Establishes linkages with health resources to provide additional health promotion programs and services.

g. Researches, coordinates, and delivers evidence-based health promotion programs that meet Administration on Aging guidelines and maintain program fidelity.

h. Manages and coordinates all aspects of subcontractor health promotion contracts including monitoring health promotion activity and providing consultation, as needed.

i. Identifies and maintains connections with health providers in the community, schools of health professions, and community organizations to plan joint health promotion programs.

j. Researches, plans, coordinates and implements other non-evidence-based, or no-cost health promotion programs and links them to senior centers, and other community settings.

k. Serves as consultant to senior community center directors/activity coordinators to assist them in implementing health promotion and healthy aging programs.

l. Participates with other PCA department staff and other staff from outside institutions/agencies in the design, development, and implementation of new services and programs. This may include participation in grant writing, contract development, proposal review, program evaluation, etc.

m. Assists in annual Senior Farmer's Market produce voucher distribution as needed.

n. Performs other duties as assigned.
