JOB TITLE: School Social Worker - District Elementary Connections Program at Linden Street School
REPORTS TO: Special Services Administrator/ Building Principal
PRIMARY ROLE:
The School Social Worker is a liaison between home, school and the community to connect and coordinate family and community support services, while supporting students' academic and mental health needs at school. They advocate for students and assist them in reaching developmentally appropriate milestones. $150 per day for the first 40 days and then placed on the appropriate step in the Teachers Bargaining Agreement.
PERFORMANCE RESPONSIBILITIES: Provides intensive case management/care coordination for students for students with emotional and behavioral disabilities in the district elementary self-contained prorgram
, as well as other students with complex mental health needs as assigned by the Special Services Administrator and Building Principal Collaborates with school personnel, families and community based service providers to implement coordinated mental health and other community based supports to help students and families overcome obstacles to effective participation in school Facilitates the development of function based behavior support plans and data collection/analysis processes in collaboration with the school based behavior support team to address the needs of students with challenging behaviors that are beyond the capacity of the classroom teacher to address Provides targeted instruction and reinforcement of student problem-solving and coping skills as part of the school's multi-tiered system of supports Provides targeted supports to increase parents' understanding of the unique needs of their child and to maximize constructive family participation in the educational process Develops and implements crisis intervention strategies and family supports for students in crisis Serves as liaison with such outside agencies as D.C.F., Juvenile Court, and the like, as appropriate. Makes home visits for the purpose of gathering helpful information on a student's background as appropriate Documents all activities with students, families and school personnel as a record in school-based interventions and supports QUALIFICATIONS AND SKILLS:
Maintains appropriate Connecticut Certification as a School Social Worker (#071). LMSW or LCSW preferred. Demonstrates strong diagnostic skills to address the mental health needs of students and develop effective treatment plans to address obstacles to school functioning and creative solutions to challenging situations, including the development and implementation of IEP goals for students with counseling services needed to access their special education instruction. Demonstrates effective communication skills, both verbally and writing, in order to effectively facilitate discussions about difficult topics and to document supports and interventions with supporting details, including written data collection and analysis Skilled at prioritizing daily workload to meet the needs of the school community, including setting appropriate boundaries for requests outside the scope of the position Knowledgeable about the resources and supports available within the greater Plainville community Empathetic yet objective in order to maintain student wellbeing as a top priority
EVALUATION: Performance of this job will be evaluated in accordance with provisions of the Board's policy on Evaluation of Professional Personnel.
