Compensation Range: $18.10/hr - $25.74/hr

Essential Duties
- Attends weekly meetings and regularly meets with the Clinicians to determine the treatment and needs of each child for case management services; coordinates case management services to ensure the needed services are provided.
- Schedules Child and Family Treatment Meetings; ensures that all adjunctive program reports are received by the treatment plan review date in order to ensure that communication regarding the child/families treatment is being represented at that meeting, and that necessary documentation is completed; inputs assigned reports into the client record.
- Ensures that client visitation schedules are prepared on a weekly basis and routed to the appropriate treatment team members.
- Makes necessary contacts with treatment team members (i.e., attorney, CSW, family members, agency volunteers, CASA Worker, auxiliary treatment programs, school personnel, etc.) as assigned by the Clinician; documents all contacts in the child's Case Activity Note.
- Makes regular and timely phone contact to treatment team members regarding the notification of Significant Incident Reports.
- Provides court ordered monitoring of visits between the children and family member(s); transports children to other sites in order to facilitate family visits; provides feedback regarding the visitation to the assigned Clinician; completes all necessary documentation of the visit in the Case Activity Note.
- Completes all Needs and Services Plans accurately and in compliance with DCFS timelines, ensuring that all contributing departments complete their assigned sections of the NSP by the deadline.
- Assists in the regular scheduling of psychiatric consultation appointments for each child; initiates and tracks to completion the necessary documents to obtain Psychotropic Medication Authorization.
- Assists the Clinician when hospitalizations occur in ensuring needed communications with DCFS, DMH, family, etc., are completed, and properly documented.
Qualifications
To perform effectively in this position, the incumbent must have:
- Completion of a Bachelor's degree in the behavioral sciences preferred, especially one related to children.
- Knowledge of Department of Mental Health (DMH) documentation
- Effective communication skills both oral and written; bi-lingual skills in Spanish/English are preferred.
- Computer skills sufficient to meet demands of the job including Microsoft office and WELLIGENT software for program notes. Skills in maintaining detailed, accurate, and complete records.
- Ability to be flexible and work in a changing environment, work varied hours including evenings, nights, and weekends, and effectively as part of a team.
- Must have a valid California Driver License, proof of automobile insurance and meet the Agency's safe driver guidelines.
- Sensitivity to the different cultures represented among the clients, families, and staff.
- Basic understanding of age-appropriate behavior for children/adolescents.
- Knowledge of emergency procedures of the agency.
- Ability to model positive behaviors and agency values, strong leadership and problem-solving skills.
- Ability to work effectively as part of a team
