Position Summary Licensed Practical Nurse II delivers patient care listed under physician or RN direction. The role utilizes the Nursing Process and Relationship‑Based Care model, integrating Shared Leadership and Evidence‑Based Practice into clinical activities to improve patient outcomes.
Duties & Responsibilities
Complete required orientation and unit‑specific competencies.
Assist in patient assessment by observing, collecting, recording, and reporting data.
Contribute to nursing care plans with the registered nurse based on ongoing observations and family input.
Implement orders from physicians, PAs, or APRNs as delegated, documenting accurately in the electronic medical record.
Share information and collaborate with the health‑care team to promote effective patient care.
Recognize symptoms, establish priorities, and respond to patients’ changing conditions.
Engage patients and families to support appropriate educational plans.
Demonstrate commitment to UI Health values through teamwork and effective communication for quality outcomes and safety.
Conduct screening tests for common diseases/viruses.
Other duties as assigned.
Advocate for patients, families, and the nursing profession.
Integrate evidence‑based research, current standards, and clinical experience to provide quality patient care.
Support shared leadership as the method of nursing governance.
Participate in departmental and shared‑leadership activities; support unit goals.
Maintain licensure and certifications (LPN, BLS, unit‑specific).
Adhere to the Medical Center’s Code of Conduct.
Treat patients, families, staff, and visitors with respect and dignity.
Follow nursing time, overtime, and attendance policies.
Complete electronic learning modules and related programs by assigned dates.
Comply with TB, flu, and fit‑testing requirements.
Escalate patient‑care problems through established communication channels.
Identify recurring clinical issues and participate in problem‑solving.
Collaborate with nurses to provide learning experiences for staff and students.
Serve as a resource for nursing staff and students.
Use the Medical Center intranet for policy changes and practice updates.
Support nursing and multidisciplinary research efforts.
Engage in professional growth by identifying learning needs and initiating strategies.
Reporting Structure This position operates within the scope of practical nursing as defined by the Illinois Nurse Practice Act under physician or RN direction. The Director acts as coach and mentor, providing guidance and feedback. Performance is evaluated annually and more frequently if needed.
Minimum Qualifications
Graduation from an accredited LPN program.
Eligibility for licensure as an LPN by the Illinois Department of Financial and Professional Regulation.
Valid Illinois LPN license.
Six months of nursing experience in a health‑care facility.
Preferred Qualifications None listed.
