Position Overview
The Health Promotion Specialist is a key member of the Chronic Disease Prevention Program in the Prevention Services division of the Gallatin County–City Health Department. The role supports initiatives designed to improve longevity and quality of life for people at risk for or diagnosed with chronic conditions. The specialist prepares people to make informed choices, promotes early diagnosis and management of chronic conditions, and collaborates with community providers, partners, and health care agencies to implement health promotion activities. The position reports to the Prevention Services Division Director and is grant‑funded.
Responsibilities
- Coordinate the deliverables of the Coordinated Chronic Disease contract, including developing annual work plans, submitting quarterly and final reports, and preparing evaluation summaries.
- Assess the need for prevention and control of chronic disease and injury in the jurisdiction, including for populations disproportionately impacted.
- Evaluate the social determinants of health and their impact on chronic disease and injuries.
- Convene public health partners to identify strategies for governmental action and develop collaborative plans for prevention and control.
- Implement population‑based strategies for chronic disease and injury prevention, providing direct service delivery and case management as needed.
- Influence policy, systems, and environmental changes to prevent harm and improve health outcomes.
- Plan, organize, coordinate, and participate in the development and evaluation of evidence‑based health promotion and education programs.
- Respond to inquiries, locate resources, and connect individuals and partners to appropriate staff or agencies.
- Utilize customized software to collect, document, query, and report on program data.
- Serve as a health education resource, applying research principles to program design.
- Develop and maintain strategic partnerships with governmental and non‑governmental partners, including those in historically marginalized communities.
- Collaborate with partners to co‑create communication strategies and coordinate routine chronic disease prevention communications.
- Provide timely, scientifically accurate, and locally relevant information on chronic disease and injury prevention.
Other Duties
Perform a variety of other duties as assigned by the supervisor, including special projects, occasional coverage for other employees, participation in ongoing trainings, and emergency response duties as outlined in departmental plans.
Qualifications
Applicants must have knowledge of community resources, adult education techniques, health education principles, health care systems, communication, marketing, and public relations. Proficiency in standard computer and software systems, excellent grammar and spelling skills, and strong written and verbal communication abilities are required. Candidates should be extremely organized, detail‑oriented, and capable of cultivating relationships with health care providers. Customer service standards and the ability to remain calm under stressful situations are also essential.
Education & Experience
Bachelor’s Degree in health education or a related field is required. One (1) year of related experience in education and/or public health education is preferred.
