across Maricopa County , we’re looking for someone who thrives on building genuine partnerships and isn’t afraid to roll up their sleeves to make a difference. This isn’t just a marketing position — it’s a chance to help more families discover the comfort, dignity, and support that hospice care provides when it matters most. Role Overview As our Community Liaison, you’ll be the driving force behind how our mission reaches more families. You’ll craft meaningful partnerships with healthcare professionals, guide families toward compassionate care, and play a pivotal role in shaping how our community understands and accesses hospice support. You will serve as the bridge between Nueva Hospice and the community — developing partnerships with
physician offices, skilled nursing facilities, hospitals, senior living communities, and assisted living centers
to increase awareness and trust in our services. Key Responsibilities Develop and Maintain Referral Relationships:
Build trusted partnerships with physicians, case managers, discharge planners, social workers, and facility administrators throughout Maricopa County. Promote Nueva Hospice Services:
Educate and inspire healthcare partners through impactful presentations that change the way they think about hospice, leading meaningful in-services, lunch-and-learns, and educational sessions. Facilitate Seamless Referrals:
Coordinate with internal clinical and admissions teams to ensure smooth patient transitions from hospitals or care facilities into hospice services. Represent Nueva Hospice in the Community:
Attend networking events, health fairs, and senior resource expos to represent Nueva Hospice’s mission and care philosophy. Identify Growth Opportunities:
Proactively identify new outreach opportunities to expand access to compassionate care. Provide Ongoing Support:
Serve as a key point of contact for referral partners—addressing concerns, following up on feedback, and ensuring satisfaction. Collaborate with Internal Teams:
Work closely with leadership, admissions, and clinical teams to align marketing efforts with operational capacity and patient needs. Qualifications Proven ability to work independently and take initiative in a fast-paced, relationship-driven environment. A passion for building community partnerships that make a lasting difference. A self-motivated relationship builder who thrives on creating trust, cultivating meaningful partnerships, and representing a mission with authenticity and heart. 2+ years of experience in
healthcare marketing, community outreach, or business development
(preferably hospice, home health, or senior care). Strong interpersonal and presentation skills, with the ability to connect authentically with healthcare professionals and families. In-depth understanding of hospice philosophy, services, and eligibility criteria (or willingness to learn). Excellent communication, time management, and organizational skills. Ability to travel throughout
Maricopa County
daily for community visits and partner meetings. Bachelor’s degree in Business, Marketing, Healthcare Administration, or a related field preferred. Mission-Driven Culture:
