This is a full-time, in-person position, Monday - Friday 9am - 5pm. Qualifications Skilled in driving referrals and increasing census. Ability to build relationships with hospitals, case managers, and community partners. Understanding of SNF admissions criteria and reimbursement. Track record of increasing facility occupancy. Representing the facility at events and building brand awareness. Responsibilities Connect with hospitals, discharge planners, and community partners to drive admissions. Develop and implement strategies to maintain facility occupancy. Showcase the facility to potential residents, families, and referral sources. Inform referral partners about SNF care, specialty programs, and insurance options. Monitor admissions trends and referral sources for strategic planning.
