About Hospital to Home LLC: Hospital to Home LLC is a mission-driven medical transport provider focused on bridging the gap between acute care and home health. We deliver high-quality, patient-centered services that support recovery and improve outcomes for patients transitioning from hospital to home. Our EMTs play a vital role in ensuring patients receive the right care at the right time—wherever they are.

Position Summary: We are seeking a compassionate and dependable EMT to join our Mobile Integrated Healthcare team. In this role, you'll provide basic life support (BLS), safe patient transport, and assist in delivering high-quality care across a variety of settings—from hospitals to patient homes. EMTs with Hospital to Home LLC are more than transport professionals; they are healthcare advocates helping to close gaps in care.

Key Responsibilities

  • Respond promptly and professionally to scheduled and unscheduled medical transports and patient assessments.
  • Provide basic life support services, including CPR, oxygen administration, and patient monitoring.
  • Assist Paramedics and healthcare staff during assessments, treatments, and transports.
  • Operate emergency vehicles safely and in accordance with company policy and local/state regulations.
  • Maintain clean, stocked, and operational ambulances and equipment at all times.
  • Complete accurate, timely documentation of all care and services rendered using electronic or paper systems.
  • Communicate effectively with dispatch, hospital staff, patients, and families.
  • Uphold a compassionate, respectful bedside manner in all interactions.
  • Follow HIPAA guidelines and maintain strict patient confidentiality.

Qualifications

  • High School Diploma or GED required.
  • Current State EMT Certification (NREMT certification if required by state or employer).
  • Valid CPR (BLS) certification; additional certifications (e.g., EVOC, IC) are a plus.
  • Valid driver’s license with an acceptable driving record (for 3 years or more).
  • Ability to lift, move, and carry patients and equipment in various settings (125 lbs. individually and 250 lbs. assisted).
  • Excellent communication, teamwork, and customer service skills.
  • Experience in EMS, home healthcare, or patient transport preferred but not required.

Working Conditions

  • Shifts may include days, nights, weekends, and holidays.
  • Work involves physical activity, including lifting, walking, and driving.
  • Exposure to a variety of environments and weather conditions during field operations.
  • Occasional contact with stressful or emotionally challenging situations.

Why Choose Hospital to Home LLC?

  • Work with a growing, innovative healthcare team focused on people, not just protocols.
  • Opportunity to make a real difference in the lives of patients every day.
  • Supportive team atmosphere with continuing education and career growth opportunities.
  • Competitive pay and flexible scheduling.

Hospital to Home LLC is an equal opportunity employer. We are committed to building a diverse and inclusive team that reflects the communities we serve.
