This is a DIRECT HIRE OPPORTUNITY (not temp or contract).
Key Responsibilities: Provides clinical physical therapy evaluation and treatment plans for seniors within the community in an outpatient setting Responsible for integrating physical therapy treatment plans into the Plan of Care (POC) as directed by the physician orders. Collaborate with a multidisciplinary team to develop and implement comprehensive treatment plans. Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Advise patients and their families on in-home treatment options and what to expect from the recovery process
Key Requirements: Bachelors Degree in Physical Therapy required, Masters in Physical Therapy preferred Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association or other equivalent organization approved by CMS Current, active Physical Therapist license within state of practice A minimum of one (1) year experience working with the elderly
Compensation and Schedule: Pay rate starts at $85K but can be considerably higher based on years of experience Comprehensive benefits plan Monday-Friday, 8AM - 4:30PM Paid holidays and NO weekends
