Under thesupervision of the Care Coordination Supervisor, the Resource Coordinator isresponsible for identifying, assessing, advocating, and providing assistanceand information to individuals age 60+ and their caregivers. This personserves as a single entry point for agency and community services and utilizesskilled interviewing techniques to assess client needs.
POSITIONRESPONSIBILITIES
Maintain and continually update resource manualto include information pertinent to persons 60+ and their caregivers.
Accept telephone calls, walk-in visitors, andclient inquiries regarding services and resources available to persons 60+and their caregivers.
Advocate for and assist persons 60+ and theircaregivers in securing needed agency and community services.
Appropriately refer clients and/or caregiversto proper persons or agencies in the community, including Adult ProtectiveServices.
Utilize interviewing skills to completecomprehensive screenings of persons 60+ for home delivered meals, personalcare, free wills, and communityprograms.
Promptly address a variety of emergency andelder abuse prevention assistance requests.
Perform community presentations about agencyservices as requested.
Attend community meetings and trainings.
Maintain current and accurate written andelectronic client records and prepare reports as required.
Prepare for and pass AIRS (Alliance ofInformation and Referral Systems) exam in order to obtain certification as anInformational and Referral Specialist in Aging Services.
Attend departmental staff meetings and otheragency activities as requested. Perform other tasks related to the position.
ESSENTIALSKILLS AND EXPERIENCE
Education:Degree from an accredited college or university in social work or relatedfield, or equivalent experience.
Minimumof one year experience working with 60+ population.
Skilledinterviewing techniques.
Excellentoral and written communication skills.
Comprehensiveworking knowledge of new and ongoing programs and services available toassist persons 60+ and their caregivers.
Knowledgeof changing eligibility requirements for these programs.
Abilityto establish and maintain effective working relationships with clients, otherprofessionals, staff, community agencies and the general public.
Abilityto prepare reports, maintain accurate records and enter client data intoagency database.
Abilityto communicate effectively both orally and in writing.
Abilityto work independently.
SPECIALREQUIREMENTS
Driver’slicense valid in the Commonwealth of Virginia.
VirginiaNotary Public Commission helpful.
