Position: Health Promotion Coordinator

Reports To: Volunteer Program Manager

Salary: $46,800.00

Position Type: Full-Time

FLSA: Exempt

Location: Rockford, IL

Summary

This position is responsible for delivering evidence-based health promotion programs to older adults across designated counties while meeting performance indicators set by funding sources. The role includes coordinating with healthcare partners and host sites, conducting outreach, recruiting participants, and maintaining strong community relationships. Responsibilities include scheduling and leading workshops, managing participant documentation, tracking data and service units in AGINGIS, and submitting timely monthly and quarterly reports. The position requires effective communication, accurate record-keeping, regular follow-up with participants, and active representation of the agency at community and networking events. Additional duties include maintaining supplies, preparing monthly "Food for Thought" submissions, and collaborating with program leaders to ensure successful delivery of services that support the agency's mission.

Job Responsibilities

  • Deliver approved evidence-based programs and meet required performance indicators.
  • Coordinate with local health departments, hospitals, and other healthcare partners.
  • Conduct outreach (calls, emails, visits) to explain programs and schedule presentations.
  • Recruit participants, maintain attendance, and follow up as needed.
  • Research and secure presentation locations with high concentrations of older adults.
  • Prepare, distribute, and collect all required workshop materials and documentation.
  • Track service units and participant data in the AGINGIS system.
  • Submit accurate monthly and quarterly reports by required deadlines.
  • Maintain updated schedules, electronic records, and supply inventories.
  • Attend networking and service-provider meetings as assigned.
  • Build and maintain strong relationships with healthcare professionals to support recruitment and program delivery.
  • Co‑lead workshops with other trained leaders and support agency programs as needed.
  • Prepare monthly "Food for Thought" and communicate program updates to the supervisor.
  • Perform additional duties assigned by the supervisor or administration.

Minimum requirement

  • Bachelor's Degree or equivalent experience.
  • Ability to exercise independent judgment and function autonomously within established guidelines.
  • Demonstrated strong decision-making abilities, attention to detail and effective communication skills, dependability, and openness to new ideas.
  • Minimum three years of working with older adults in health or human services.
  • Ability to balance and prioritize multiple responsibilities effectively.
