Home Health Marketer / Community Liaison
Part-Time
Precise Med Home Health is expanding, and we’re looking for a motivated Part‑Time Home Health Marketer / Community Liaison to help strengthen our relationships with local healthcare providers, skilled nursing facilities, assisted living communities, and other referral partners.
This role is ideal for someone who is outgoing, professional, and passionate about building meaningful connections in the healthcare community.
Responsibilities
- Visiting clinics weekly
- Introducing our home health services
- Maintaining consistent follow‑up
- Strengthening relationships with providers and staff
- Networking with Social Services Directors, DONs, Administrators
- Dropping off materials
- Supporting discharge teams and care coordinators
- Engaging with community resources
- Connecting with senior centers, case managers, and care coordinators
- Identifying new referral sources
- Managing warm leads
- Documenting outreach activity
- Helping drive sustainable growth for the agency
- Conduct consistent field visits weekly
- Follow up with clinics, SNFs, ALFs, and other partners
- Maintain professional communication
- Track and report outreach activity
- Coordinate with intake to ensure smooth transitions of care
Qualifications
- Excellent communication & interpersonal skills
- Professional, polished, and confident presence
- Organized, reliable, and consistent
- Sales or healthcare marketing experience preferred, not required
- Reliable transportation
Location
Roseville, Rocklin, Citrus Heights, Antelope, Granite Bay, and nearby Sacramento areas.
