Position Summary

The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation. Practicing under the supervision of transplant surgeons and physicians, the transplant nurse coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout the pre‑transplant phase. The role may involve work in kidney, pancreas, liver and/or small bowel programs. The clinical nurse consultant II applies the Nursing Process and Relationship‑Based Care model to facilitate the delivery of patient care, provides patient care and consultation, coordinates staff education, and serves as a clinical role model. The CNCII strives to improve patient outcomes by integrating shared leadership, evidence‑based practice, and continuous quality improvement.

Duties & Responsibilities

Evaluates patients at the appropriate UI Health site of care.

Coordinates care for transplant recipients from referral and/or until wait‑listed, all potential recipients with living donors, and all living donor patients.

Ensures that all pre‑transplant patient coordination and patient education is documented and is in compliance with UNOS, CMS transplant program guidelines and UI Health policies/procedures.

Collaborates with the provider to order and review all testing for evaluation as needed, timely following UNOS and CMS guidelines.

Develops, coordinates, and communicates the patient’s plan of care based on ongoing assessment and family input.

Uses clinical expertise to collaborate with a multidisciplinary health care team to improve patient outcomes.

Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record.

Assesses, engages and obtains clinical information from the multidisciplinary team, patient, and/or family to develop an appropriate educational plan.

Recognizes symptoms, establishes priorities, and responds to changing patient needs throughout the transplant care continuum.

Ensures that the patient is added to the UNOS waiting list and the required listing letters are sent timely per UNOS requirements.

Coordinates and schedules all living donor transplant surgeries as assigned.

Attends and participates in transplant QAPI meetings, listing meetings, department meetings, multidisciplinary planning meetings, and other meetings as needed.

Provides cross‑coverage for the transplant data coordinator by entering living donor information in UNOS prior to the living donor surgery as assigned.

Engages nursing and support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides support and feedback.

Demonstrates a commitment to UI Health’s values through teamwork and effective communication to achieve quality outcomes, patient safety, customer satisfaction, and a clean, safe, therapeutic environment.

Integrates theoretical knowledge, evidence‑based research, current standards of practice, and clinical experience to provide quality patient care as measured by established nursing quality standards and governing entities.

Participates in organizational, departmental, and shared leadership activities; supports department and unit goals in daily behavior.

Collaborates with other nurses and physicians to identify, promote, and provide positive learning experiences for staff and students.

Initiates, conducts, and analyzes evidence‑based practice research to improve patient care outcomes; serves as preceptor, mentor, and resource to other nursing staff and students.

Other duties as assigned.

Minimum Qualifications

High school diploma or equivalent.

Graduation from an accredited school of nursing.

Eligibility for registration with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.

Two years of nursing experience in the branch of service to which assigned, with at least one year comparable to a Clinical Nursing Consultant I.

Must communicate effectively (written and verbal) in English and Spanish.

Ability to select appropriate teaching methods based on the educational and experience backgrounds of employees.

BSN (or higher) required from an accredited nursing program.

Current BLS/CPR and/or other required unit‑specific certifications.

Current advanced cardiac life support, or within 6 months of hire for the main hospital.

Nationally certified through an ANCC‑approved certifier, or within 6 months of hire in an area relevant to the clinical area.

Preferred Qualifications

Previous RN experience working with solid organ transplant patients.
