Senior Helpers of Fargo is looking to hire a Client Care Manager, RN. The Client Care Manager, RN will oversee the care of clients and monitor aides. This includes performing nursing assessments and reassessments at required intervals; writing, monitoring, and updating care plans for clients for the home care agency. This position will develop care plans and aid training specific to the client's medical conditions (i.e. CVA, MS, ALS, CHF, COPD, DM). In addition, the RN will perform skills assessments for the home care aides. The Client Care Manager, RN may also perform medication management, wound care and diabetic foot care for current clients.

Job Duties
Utilize professional assessment skills to identify physical, mental, and psychosocial needs of the client and be able to develop a care plan to meet these needs in a multi-disciplinary approach
Act on the client’s behalf to ensure client rights
Conduct home safety assessment and recommend modifications and DME to ensure the client's fall risks and other safety concerns are minimized
Has a regular communication process for new clients that may include caregiver introductions, first day of service calls, and visit(s) within the first 30 to 90 days of service
Visits clients on a systematic basis to assess the quality of services as well as the client and family satisfaction. Spends approximately two to three days out of the office reassessing clients and managing client visits in the hospital, rehabs, facilities, etc.
Adapts and communicates the client care plans as needed
Communicates with scheduler or Care Team Manager if caregivers need to be changed
Generates internal growth each month, consistent with company goals
Completes assessments and reassessments using LIFE Profile
Takes service inquiry calls and follows up with leads to meet client conversion targets
Seeks opportunities to enhance seniors’ quality of life through various service offerings
Other duties as assigned
Qualifications
Registered Nurse, licensed in ND/MN and three years of related work experience strongly preferred
Current TB TestMust maintain CEU requirements as required by the Board of Nursing
Valid Driver's License and reliable transportation
Minimum of one year experience managing clients
Previous industry experience preferred
Must have exceptional organizational skills, attention to detail, and the ability to prioritize in a changing environment
Must have excellent verbal communication skills and follow-up skills with prospects, clients, client families, and referral sources
Ability to quickly build rapport
Creative and innovative problem-solving abilities
Proactively prevent and resolve issues
Ability to visit client homes in the assigned territory
Proficiency in Microsoft Word, Excel, Internet, and Outlook required
Ability to learn other software programs quickly
Ability to work independently and as part of a team
