Residential Rehab Coordinator

Schedule Sun-Thurs (Sun, Mon, Tues, 9 a‑5 p & Thurs, Fri, Noon‑8 p)
Responsibilities
Advocate for client caseload
Develop, implement, and monitor treatment plans
Oversee medication management
Fiscal management of funds
Serve as primary liaison for service providers, family, and guardians
Utilize an electronic health record system for client notes
Perform a variety of administrative, communication, and reporting functions
Enter notes into Avatar; review notes entered by other residential staff for clarity and accuracy
Report incidents to Site Manager and other providers ensuring confidentiality
Education
HS diploma and at least 1 year of related field experience; additional education toward an AS or BA in health care is preferred.

Time‑management and ability to organize multiple priorities
Calmness and de‑escalation of stressful situations
Good judgment and problem‑solving ability
Dependability and flexibility
Team‑work orientation and role‑modeling for other staff
Ability to form therapeutic relationships with challenging clients
Computer proficiency and ability to use an electronic health record system
Valid driver’s license, safe driving record, and access to a personal vehicle
Compensation Annual salary range: $22 – $22 USD
