The
Community Engagement Team
is designed to conduct outreach and in-reach to identify and engage high acuity clients to be seen for intake and opened as new client at CCBHC programs.
The
Certified Recovery Mentor
providescase managementoffering a combination of referral linkage, individual counseling, familysupport, group counseling and case management services to assist client'ssuccessful connection and engagement in treatment. Providesservices primarily offsite (within the community and at community partnerlocations).
Location:
This position is based out of LifeWorks NW's Rockwood site.
Pay/Benefits : $22.74-$29.26 per hour plus Full benefits . Salary Placement Notice: The posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. However, more seasoned professionals with extensive exact or similar experience may be considered for placement toward the higher end of the range. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws.
ESSENTIAL RESPONSIBILITIES:
Screensand selects clients to participate in the mentoring program.
Documentand maintain client records in electronic healthcare system within 24hours of meeting with client to ensure compliance with agency and contractguidelines and confidentiality.
Meetsregularly with staff from social service agencies to update them onLifeWorks NW services identifying unmet services/needs LifeWorks NW may beable to address; review and update referral and case coordinationprocedures as needed; obtain referral requests and work with referralsources to remove any barriers to treatment; report back if clients failsto attend; obtain relevant case information from the referral for theclinician who will be doing the client's intake assessment; work withreferral source and client to facilitate the family's involvement in theassessment and the treatment planning process.
Allservices must be provided in a culturally appropriate manner.
Travelsbetween sites or in the community driving clients when needed.
REQUIREMENTS:
Musthave a current Certified Recovery Mentor Certificate (CRM) with MHACBO.
Substantialexperience with issues involving addiction including the language, values,norms and other attributes of the subculture; the subtleties andlinguistic nuances of the differences between honest dialogue andstatements made for effect.
Theability to maintain confidentiality and appropriate boundaries.
Extensiveknowledge of how to efficiently navigate the social service network fromthe bottom up.
Mustbe able to accompany clients throughout their environment.
Musthave knowledge of the legal/corrections system.
Demonstrateseffective communication skills and maintains a cooperative workingrelationship with others in a culturally diverse environment.
Proficient in Microsoft Outlook and Word.Ability to master proprietary software including electronic health recordsystem and scheduling system required.
Requiresability to work some evenings and/or weekends
Requiresaccess to car, valid driver's license, proof of current automobileinsurance coverage and verification of safe driving record from DMV.
