Role Overview Family Service Coordinators provide case management/service coordination for children ages zero to three who show a developmental delay or other atypical development. Responsibilities include conducting screenings, facilitating eligibility determination, organizing and participating in evaluations, developing care plans, linking families with community resources, coaching families to be effective advocates, and arranging services needed at discharge. Coordinators must demonstrate excellent documentation, organization, and customer service skills.
Key Responsibilities
Conduct screenings for children ages 0-3.
Facilitate eligibility determination and organize evaluations.
Develop individualized care plans.
Link families with community resources.
Teach families advocacy skills.
Arrange services needed at discharge.
Maintain accurate documentation and records.
Skills & Qualifications
Fluent in Spanish.
Knowledge of the local service delivery system, target group needs, and available resources.
Bachelor’s Degree in a human services related field.
At least one year of supervised experience working with families with special needs (preferred).
Valid Wisconsin driver’s license, reliable transportation and car insurance.
Experience and passion for working with children ages 0-3.
