Responsibilities Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
Qualifications
Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program OR Bachelor of Science (B.S.) AND a minimum of 2-years relevant experience.
Preferred Qualifications
Other advanced degrees (e.g., Master’s, Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor.
Completion of pharmacy residency and specialization (e.g., PGY-2 and respective BPS certification), if applicable.
Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations.
Relevant applicable pharmacy experience working in a progressive and collaborative practice environment.
Experience and/or interest in teaching andcepting.
Additional Qualifications
Commitment to excellence, accuracy, attention to detail and teamwork required.
Ability to communicate effectively, work collaboratively with others, organize time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
License and/or Certification
Current pharmacist license by the Minnesota Board of Pharmacy, or eligible to obtain within 60-days of start date. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
Employment Details Full Time
Schedule: Monday - Friday, 8:00 am - 5:00 pm (No weekends)
Hours/Pay Period: 80
Compensation: $145,000 - $185,000 annually
