The Director of Community Support Services will be responsible for leading and inspiring the development of the Gender Health Center’s Community Support Services (CSS) department, which provides comprehensive case management, peer healthcare navigation, resource referral, cultural programs, and supportive services alongside Two-Spirit, transgender, gender diverse, and intersex people and the broader community. The Director of CSS will provide supervision and direct support to CSS staff team members, volunteers, and trainees to ensure quality care services, and engage in ongoing program development and evaluation.

Services provided by GHC include a free hormone clinic, mental health counseling, Mental Health Counseling Training Program, support groups and culturally specific community programs, HIV and STI testing and linkage to HIV healthcare, mobile harm reduction services, a hosted electrolysis clinic, legal assistance, advocacy, and resource referrals.Through its advocacy efforts, the GHC has played a significant role in raising awareness about trans healthcare inequities and promoting policy changes to improve the rights and well-being of transgender community members. GHC has actively engaged with legislators, community leaders, and healthcare institutions to advocate for access to gender-affirming care, trans-inclusive policies, and equitable access to services. Description What You’ll Do The general and specific job responsibilities include but are not limited to:
Team Leadership and Supervision
Provide leadership, direction, and task support to the Community Support Services team, including the Community Care Manager, CSS Specialist(s), volunteers, and trainees/interns.
Foster a collaborative, supportive, and healing-centered work environment that encourages growth, professional development, and mission alignment.
Conduct regular supervision meetings and complete on time semi-annual performance reviews to support team effectiveness, morale, and overall wellness.
Model emotionally intelligent leadership by holding tension, addressing conflict directly and constructively, and supporting staff through transition and uncertainty.
Provide clear expectations, timely coaching, and constructive feedback.
Attend professional development opportunities that are offered by grant funders.
Stay up-to-date with best practices, policies, and legislation related to gender affirming care and healthcare access, and incorporate this knowledge into your work.
Participate and facilitate team meetings, training sessions, and professional development activities to enhance skills and knowledge base of the team.
Provide quarterly reports of CSS progress and projects to the executive team.
Create, refine, and maintain data management systems to ensure accurate, efficient, and confidential reporting.
Attend and lead portions of GHC Staff Gatherings/Kikis and retreats.
Serve as a liaison and task and clinician supervisor for BSW and MSW I graduate students.
Interview and screen potential BSW and MSW-I student trainees for suitability of social work placement at the GHC.
Ensure GHC values are reflected in leadership decisions, including budgeting, hiring authority, and program evaluation.
Assess the needs of our community members in the Greater Sacramento area and in rural parts of Northern California.
Develop and supervise innovative cultural programs and services to address those needs.
Collaborate across departments to support interdisciplinary initiatives and foster in-house referrals and capacity building.
Collaborate closely with peer Directors and the Executive Director to align strategy, share accountability, and ensure organizational priorities are advanced consistently across departments.
Collaborate with the executive team to define strategic goals and objectives for the CSS department.
Stay informed about best practices and emerging trends in transgender and gender diverse healthcare and support services.
Complete and submit in a timely manner grant reports to support CSS funded projects.
Be the point of contact for grant managers for CSS funded projects.
Ensure department attendance at grant sponsored training and meetings.
Collaborate with the Executive Team to ensure that cultural programs and events stay within budget.
Work with the Director of Finance and Administration to establish annual departmental budgets.
Community Engagement & Advocacy
Establish and maintain a network of community partners and resources to provide comprehensive referrals to community members.
Maintain and strengthen our medical-legal partnership with Legal Services of Northern California and partnership with One Community Health through continued collaboration and communication.
Ensure community members are connected to necessary services, including mental health counseling, housing assistance, legal support, nutrition support, and more.
Monitor the quality and effectiveness of referrals and follow up with community members as needed.
Build and maintain strong relationships with local organizations, healthcare providers, and community members.
Represent GHC at community events, city and county initiatives and coalitions, meetings, and conferences to promote awareness of our programs, care delivery approaches, and care coordination with partners.
Support and help to curate the development of social media, newsletter, and website content to educate our community and partners about relevant healthcare access information.
Advocate for the needs of 2TGI people across identities and the lifespan.
Direct Services- Brief Counseling & Case Management
Deliver low-barrier counseling, case management, and crisis intervention at mobile outreach sites (once per month) and during Haus of Hormones programming.
Provide direct outreach on our mobile unit at minimum once per month.
Oversee care management across mobile and brick and mortar services.
Conduct initial assessments to understand community members’ needs, goals, and challenges.
Develop individualized case plans in collaboration with community members, outlining specific steps and resources required to achieve their objectives.
Take on more complex care management tasks.
Provide support, guidance, and advocacy to community members, acting as a trusted ally throughout their journey.
Assist community members in accessing essential services, including healthcare, mental health support, housing, legal aid, employment resources, and social programs.
Collaborate with community organizations, medical professionals, therapists, and other stakeholders to coordinate services and referrals for community members.
Monitor and evaluate community members’ progress, adjusting case plans as needed to ensure goals are being met.
Maintain accurate and confidential community member records, documenting all interactions, referrals, and progress updates.
Required Qualifications
Licensed Clinical Social Worker (LCSW) or Associate Clinical Social Worker (ACSW) with demonstrated strong case management experience.
At least 4 years of supervisory, management, or executive experience is required.
Demonstrated experience working with transgender, gender diverse, and intersex peoplein community or clinical contexts for at least 4 years.
Familiarity with community resources, healthcare systems, and support services that offer gender-affirming care and support for Two-Spirit, transgender, gender diverse, and intersex communities.
Demonstrated expertise in micro, mezzo, and macro social work practice, integrating direct clinical care, case management, supervision, program leadership, and systems-level planning and advocacy.
Preferred Qualifications
Licensed Clinical Social Worker (LCSW)
Excellent communication and interpersonal skills, with the ability to establish rapport, build trust, and maintain professional boundaries with community members and staff team.
Strong organizational skills, with the ability to prioritize tasks, manage time effectively, and work independently.
Empathy, sensitivity, and non-judgmental approach in supporting individuals from diverse backgrounds.
Knowledge of and commitment to social justice, diversity, and belonging principles.
Familiarity with relevant legislation, policies, and best practices related to transgender, gender diverse, and non-binary folks.
Demonstrate the ability to research, evaluate, and maintain up-to-date knowledge of resources in the immediate geographic area as well as across California for community member and organizational support.

The building does not have an elevator, and the 2nd floor of the building is not accessible via wheelchair or other mobility aids.
This position will require entering and exiting a van that is not wheelchair or mobility aid accessible to provide harm reduction and community support services. Please keep in mind that we run our outdoor services even in extreme heat in the summer.
COVID-19 and Respiratory Illness Policy This policy applies to all GHC employees, trainees, and volunteers.
1. All employees, trainees, and volunteers coming in-person to Gender Health Center’s building must wear a face covering.
2. All employees, trainees, and volunteers coming in-person to Gender Health Center’s building must be fully vaccinated. Fully vaccinated means receiving a COVID-19 vaccine within the last 12 months of hire.
3. All employees, trainees, and volunteers coming to the building in-person must provide copies of their vaccination records.
4. Any COVID-19 symptoms experienced by any employee, trainee, or volunteer must be reported to their direct supervisor. If symptoms occur before work they must contact their supervisor before entering the office. If symptoms occur in the workplace they must contact their direct supervisor for instructions on how to proceed.
5. If GHC becomes aware of someone who has been in the building testing COVID-19 positive, anyone who has been in the same indoor space for 15 minutes with that person will be notified.
6. All employees, trainees, and volunteers who have a close contact or COVID-19 exposure, are required to take a test.
7. If the test is positive for COVID-19, they may return to work 10 days after the original test was taken if they test negative on two consecutive tests and no longer have symptoms.
