$19.23 – $21.63 an hour
Position Summary The Children's Home (CHWC) is partnering with Care Compass Collaborative (CCC) to provide Social Care Navigation services within Broome, Chenango, Tioga, & Tompkins counties. Social Care Navigators (SCNs) play a vital role in enhancing community health networks and reducing health inequities amongst Medicaid Members, including children, caregivers, siblings, and other eligible family members.
SCNs are responsible for screening Medicaid Members for unmet Health Related Social Needs (HRSNs) including Housing/Utility; Food Security; Transportation; Employment; Education; and Interpersonal Safety. SCNs also screen Members for eligibility of community-based resources, State, and/or Federal resources. If Members qualify for any service meeting their HRSNs, SCNs navigate and connect them with such resources. Once connected, SCNs communicate with the Member to ensure Closed Loop Referrals have been completed to best meet Member needs.
Responsibilities
Provide comprehensive Social Care Navigator (SCN) services, including screening & navigation services to eligible Medicaid Members via phone, in-person, & virtually.
Utilize the UniteUs Database in conjunction with the Accountable Health Communities (AHC) Health Related Social Needs (HRSN) Screening Tool to screen Medicaid Members with a variety of HRSNs.
Conduct Eligibility Assessments for eligible Medicaid Members whose initial screening identifies unmet HRSN needs.
Connect eligible Medicaid Members to services, care providers, & community resources to ensure their HRSNs are met.
Develop Social Care Plans for eligible Members, including a summary of needs, eligibility, individualized strategies, interventions & an up-to-date list of navigated resources &/or services.
Ensure Closed Loop Referrals by following up with eligible Medicaid Members within five business days after service completion to verify navigated resources were successful in meeting Member needs.
Ensure any eligible Member enrolled in a Children's Home program, as well as any eligible Member living in the same household, are reached for screening & navigation services.
Utilize Motivational Interviewing techniques to engage Members, providing the opportunity to speak openly, safely, & honestly communicate about their unmet needs.
Communicate with local, state, &/or federal HRSN entities to form working relationships with providers & personnel; keeping up to date with navigation resources available to eligible Medicaid Members.
Hold an open & flexible schedule to provide SCN screening & navigation services as needed for Member accessibility & convenience, including evenings & weekends as needed.
Requirements Education
Associate's Degree in health, human services, or related field required
Bachelor's Degree in health, human services, or related field preferred
Experience
One year of work experience in healthcare, community-based services, &/or social welfare domains required
In-depth knowledge of community-based, state, & federal resources available to eligible Medicaid Members preferred
Experience working with at-risk populations preferred
Exemplary communication skills for effective interactions with agency staff, Medicaid Members, children, families, & the community
Exemplary written & verbal communication skills
Comfortability with heavy documentation
Driver's License and ability to maintain insurability throughout employment required
