Pay range: $27.50/hr - $32.00/hr (Base payp>
Location: 6043 Hollywood Blvd., Los Angeles, CA. Schedule: Monday‑Friday, 2 days in office, 3 days remote.

Duties and Responsibilities
Coordinate with individuals and/or entities to ensure a seamless experience for the member and prevent duplication of services.
Oversee provision of ECM services and implementation of the Care Plan.
Offer services where the member lives, seeks care, or finds most easily accessible and within Managed Care Plan (MCP) guidelines.
Connect member to other social services and support needs, such as transportation and housing.
Advocate on behalf of members with health care professionals.
Use motivational interviewing, trauma‑informed care, and harm‑reduction approaches.
Coordinate with hospital staff on discharge plans.
Accompany member to office visits, as needed and according to MCP guidelines.
Monitor treatment adherence (including medication).
Provide health promotion and self‑management training.
Inform patients about other services offered by Saban Community Clinic that may be appropriate for the patient’s family members.
Perform other duties as assigned by Supervisor.
Position Requirements
Bachelor and/or master’s degree in social work or related fields or equivalent experience.
3 years of prior case management experience.
Valid California driver’s license and insurance.
Related work experience in public health, health outreach, and education.
Fluency in another language such as Spanish, Russian, or Korean preferred.

The range is subject to change.
