POSITION GOAL Provides oversight and leadership for administrative actions under the supervision of the ACT Therapist. Works with ACT Therapist and Director of Community Support Services to ensure that appropriate assertive community based services are provided to individuals who have higher rates of hospital admissions, arrests, involvement in the criminal justice system and have other higher needs.

LOCATION/TRAVEL/WORK CONDITIONS AND CONSIDERATIONS This position is based at a central location; assigned duties may require activity at various community sites or business locations. Travel may be required for a variety of business reasons such as meetings, conferences, community outreach, education, etc. It is possible for the need to transport a client in own vehicle.

Position Responsibilities

Work with the Prairie View Assertive Community Treatment (ACT) team to develop and provide quality treatment for service participants and their families.

Participate in ACT program development, improvement, and evaluation as requested.

Comply with policies and procedures for medical records.

Provide appropriate, person-centered interventions to service participants as needed.

Provide all service duties associated with all funded programs in a timely manner.

Complete DLA-20 assessments, PHQ-9 assessments, Columbia/Risk Subsequent assessments, and/or ACT Triage assessments as needed.

Attend clinical staffings and department meetings as expected for work area.

Attend and participate in daily huddles with ACT team.

Provide accurate billing information.

Communicate regularly with referral sources at admission, as needed throughout treatment, and at termination.

Work cooperatively with Utilization Review staff and treatment team to assure specific services are identified on the active treatment plan.

Provide supervision to ACT team members on a regular and consistent basis.

Communicate program updates to ACT Therapist and Director of Community Support Services.

Time will be split as following: 30% clinical work/70% administrative work.

Insure that medical records are completed, as standards require: scheduling calendar is completed accurately within 24 hours; progress notes are completed within 48 hours; other duties as assigned by supervisor.
