Community Health Coach – WeCare Location: Southwest Health Center – 1522 W Morris Street, Indianapolis, IN 46221.

Fair Labor Standards Act Classification: Non-Exempt.

Position Type: Full-time.

Grant funded position with a time limit. The role is in person.

What You’ll Do WeCare is a program designed to reduce risk factors for infant mortality through two-way SMS messaging and health coaching. The program focuses on decreasing smoking, obesity, and mental health issues while increasing safe sleeping practices and breastfeeding.

As a Community Health Coach, you will support and coach women and men enrolled in the WeCare Program by:

Providing direct crisis intervention, short‑term counseling, information and referral, and psychosocial assessment as necessary.

Collaboratively planning and facilitating interventions that improve overall family health and well‑being.

Acting as a liaison between the patient and healthcare provider regarding broad psychosocial aspects of the patient's treatment.

Taking appropriate action according to established protocol for child and adult abuse and neglect cases.

Applying a minimum of one health literacy method when communicating with patients/clients.

Charting & Record Keeping

Charts daily appropriately and in a timely manner in patients’ medical records.

Participates in multidisciplinary patient case conferences designed to share pertinent information.

Education & Training

Provides consultation and educational services to patients, families and community groups relevant to needs and problems.

Requirements & Qualifications

Currently not sponsoring work visa.

High School degree (college degree preferred).

Experience working with patients and families in clinic, hospital, home, or community settings; extensive child care experience.

Excellent interpersonal skills; maintain confidentiality; logical thinking and decision‑making skills.

Teaching and coaching skills; proficient computer skills.

Transportation to residential, inpatient, and community settings.

Personal knowledge of Marion County.

Travel may be required.

Physical Requirements

Prolonged standing, walking, sitting, bending, squatting, and stooping.

Ability to lift, push, and/or pull equipment, light to moderately heavy weight up to 20–30 pounds.

Ability to perform duties during varied and/or prolonged work hours.

Exposure to all patient elements.

Must read, write, hear, and communicate effectively in English.
