Under the direction of nursing leadership, the Home Health Coordinator provides essential administrative and operational support to the home care department. This role plays a key part in ensuring high-quality, compassionate home care services through effective coordination, communication, and oversight of daily operations. It is an excellent opportunity for professionals passionate about supporting elderly or frail patients in a collaborative healthcare environment.
Duties
- Facilitate and coordinate home care services for patients to ensure timely, effective support
- Assist with the supervision and evaluation of caregivers
- Oversee general home care operations to support quality and continuity of care
- Communicate clearly and professionally, both verbally and in writing, with providers, patients, family members, and the team
- Provide administrative support to the nursing department as needed
- Perform other related duties as assigned
Required Qualifications
- Minimum of one (1) year of relevant work experience
- Prior experience working with elderly or frail populations is required
Knowledge, Skills, and Abilities
- Excellent verbal and written communication skills
- Demonstrated leadership ability, creativity, and flexibility
- Strong interest and proven ability to work with elderly or frail patients
- Ability to work independently and collaboratively as part of a team
- Basic computer proficiency, including Microsoft Word and Excel
- Working knowledge of basic medical terminology

- Experience supporting home care or community-based healthcare programs
- Prior exposure to caregiver coordination or healthcare operations
- Strong organizational and problem-solving skills
