Community Health Outreach Worker I (Full-Time and Part-Time Contractual Positions Available)
Location: Anne Arundel County, Prince George’s County, Howard County, or Montgomery County, Maryland.
Position Summary This field position requires residence in the specified counties. The Community Health Worker (CHW) promotes healthcare equity and addresses social determinants of health for Maryland Medicaid members. CHWs connect members with community resources (e.g., locating health‑care providers, arranging transportation, linking to food pantries), provide member education on health care and benefits, and identify ways to improve access to care. The role includes outreach to members, collaboration with care management, and completion of documentation and telephone outreach from the home office for the remainder of the time.
Responsibilities Outreach members to assess recovered needs and provide guiding resources; provide education related to health care and available benefits; identify opportunities to improve access to care; collaborate closely with the care management department; complete documentation and telephonic outreach from the home office when not in the field.
Required Qualifications
3–5 years of community‑related experience
Candidates must reside in Anne Arundel County, Prince George’s County, Howard County, or Montgomery County
Must be able to travel up to 70% of the time
Reliable transportation to meet with members in the community (mileage reimbursed per state guidelines)
Experience working with Medicaid populations in urban or rural environments
Proficiency with computers/technology
Previous CHW experience
Previous healthcare‑related experience
Preferred Qualifications
1–2 years of experience providing clinical services or in a community/outpatient setting
Community Health Worker Certification or equivalent training and experience
Mental Health First Aid, Trauma‑Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions
Meet all requirements set forth by the local state (e.g., first aid, CPR, TB screen)
Bilingual Spanish
Experience dealing with maternity population
Education
High School Diploma or GED (minimum requirement)
Anticipated Weekly Hours 40 hours per week.
Time Type Full time.
Pay Range The Typical Pay Range For This Role Is $18.50 − $38.82 per hour. The actual base salary will depend on experience, education, geography, and other relevant factors. This position is eligible for a CVS Health bonus, commission, or short‑term incentive program in addition to the base pay range listed above.
