Position Summary Health Home Care Managers at the Children’s Home provide strength-based, youth-focused services to help eligible children access medical and behavioral healthcare, social services, education, housing, and other essential supports. Working in partnership with the child, their family, and service providers, they coordinate the most appropriate resources to meet the child’s needs, improve well-being, and support independence in the least restrictive environment. Comprehensive Care Management uses a team-based approach to create a single care plan that ensures appropriate, high-quality, and cost-effective support. It also provides oversight and coordination to make sure health and social services are delivered effectively.
Responsibilities
Model team standards daily and actively participate in teamwork across departments.
Maintain strong relationships with DSS, community agencies, schools, and service providers to advocate for clients.
Complete required reports and documentation promptly, ensuring accuracy, professionalism, and compliance with all standards.
Provide comprehensive care management, including case planning, coordination, and crisis response services.
Develop and maintain therapeutic relationships with families, promoting parent-child bonding and family-centered practices.
Assess family strengths and needs to support intervention planning, skill development, and independent living.
Utilize client-centered approaches and implement innovative best practices based on current research and trends.
Continuously monitor, evaluate, and improve clinical programming to enhance service effectiveness.

Requirements Education
Associate’s Degree in Human Services or related field required
Bachelor’s Degree in Human Services or related field preferred
Experience
3 years of experience with case management with an associate’s degree
2 years of experience with human services with a bachelor's degree
At least two years of experience in youth-focused human services or related field required
Driver’s License and ability to maintain insurability throughout employment required
