Job Path supports autistic people and individuals with developmental disabilities, encouraging them to explore what they want out of life and to chart their own journeys. The Supported Employment program seeks an energetic and creative individual to work with participants and customize jobs for the people we support.

Responsibilities

Providing initial and ongoing support to a caseload of 10–12 people.

Conducting a discovery process to learn strengths, interests, and needs of an individual through home/community visits and interviews.

Creating a vocational profile and portfolio for participants to present to potential employers.

Networking with personal contacts and circles of support to expand connections with potential employers.

Researching businesses in NYC connected to participants’ interests.

Establishing and facilitating new partnerships.

Working with employers and organizations to customize jobs that meet employer needs and capitalize on participants’ skills.

Providing on‑job training and support to participants at and off their job sites.

Providing ongoing job coaching support to individuals with significant developmental disabilities at their worksites.

Serving as a liaison with employers to ensure satisfaction with Job Path’s services.

Documenting all services rendered and ensuring regulatory requirements are met.

Documentation and Billing

Completing a daily hours chart to document all services provided.

Submitting detailed case notes for all billable services daily into the electronic record‑keeping system.

Completing a monthly summary detailing services and progress for each person on a caseload.

Attending and providing feedback at Life Plan meetings twice a year for each person we support, creating and submitting a Staff Action Plan.

Qualifications

Bachelor’s degree and at least one year of experience working with individuals with disabilities or in job development.

Excellent written and verbal communication skills.

Experience with administrative work and record keeping.

Demonstrated ability to communicate effectively both externally and internally with team members.

Ability to actively listen and collaborate.

Competency in written, verbal and computational skills to document records in accordance with program and OPWDD mandates.

Salary Annual salary of $43,000–$45,000

(non‑exempt).
