Social Worker (MSW) – Agape Care Group
Join Agape Care Group as a hospice medical social worker and help patients receive the compassionate care they deserve.
Key responsibilities :
Conduct psychosocial evaluations and develop comprehensive care plans.
Provide ongoing counseling and emotional support to patients and families throughout their end‑of‑life journey.
Collaborate with interdisciplinary teams to coordinate palliative and hospice care.
Document progress and maintain accurate, timely records in accordance with the plan of care.
Qualifications :
MSW degree from an accredited program approved by the Council of Social Work Education.
Current state license as a social worker.
Minimum 2 years of clinical experience, preferably in healthcare or hospice settings.
Reliable transportation and the physical ability to lift at least 25 lbs and perform the duties required by the role.
