Position Summary Premier Community Health, a division of Fidelity Health Care (FHC), provides comprehensive health and wellness services to employer worksites and the community. This position requires travel to various sites in Southwest Ohio for health and wellness events. Community Health Screeners work at events to provide health screenings, results counseling, and/or vaccinations (flu and VFC) to employees and community members. They perform fingerstick tests to determine health measurements and provide counseling on results. The role also includes serving as a visit nurse for home care services, coordinating care per physician orders, documenting care, and communicating patient information to the healthcare team.

Responsibilities

Conduct health screenings, flu and VFC vaccinations, and fingerstick blood tests at community events.

Provide results counseling and quick health advice for participants.

Travel to various sites across Southwest Ohio as required.

Coordinate home care services as a visit nurse, following physician orders and policies.

Document all patient care activities and maintain accurate records.

Communicate pertinent patient information to the healthcare team.

Qualifications

Education:

Associate degree in Nursing (or Bachelor’s) from an accredited nursing program.

Licensure/Certification:

Current Ohio Registered Nurse license in good standing; CPR certification.

Experience:

1‑3 years of job‑related nursing experience; preferred one year of acute‑care clinical experience.

Knowledge & Skills:

Proficient in fingerstick testing, blood pressure screening, and using health‑screening equipment.

Capable of standing, stooping, bending, and lifting up to 40 lbs; able to walk moderate distances.

Self‑directed with the ability to work with minimal supervision.

Strong interpersonal, customer‑service, and people‑skills; reliable and professional under pressure.

Proficient in basic computer use (e.g., analytics software, Epic). Must maintain confidentiality of participant information.

Reliable transportation, accurate contact information, and a valid Ohio driver’s license.
