Join our dedicated team as a Community Care Coordinator and help us deliver exceptional care to our residents.
What You’ll Do:
Provide leadership to the Care Professionals providing services in the community.
Create and maintain client-Care Pro schedules with an emphasis on efficiency, high quality matches and extraordinary relationships.
Monitor client progress and care needs, and adjust care plans as needed.
Be a consistent point of contact for for clients and family members, from the first inquiry, through their tenure as our client.
What We’re Looking For:
Strong communication and organizational skills.
Ability to work independently.
Empathy, patience, and a genuine desire to help others.
Fantastic interpersonal skills, sound judgment and decision making skills.
Ability to multitask and manage priorities in a dynamic environment.
Basic computer skills, and a desire and drive to learn the programs we use, including Word and Excel.
Requirements:
Two years’ experience caregiving in a professional capacity.
One year of supervisory experience preferred.
Must possess a valid driver’s license.
Must be able to pass background check.
Must be able to work rotating weekends.
