Pre-Admission Screening & Resident Reviewer (MetroWest Region in MA) Join the ForHealth Consulting team at UMass Chan Medical School as a Clinical Reviewer II. Under the supervision of the Associate Director, you will conduct Pre‑Admission Screening Resident Reviews (PASRR) Level II evaluations, ensuring individuals with mental illness receive appropriate in‑person assessments and nursing facility placement determinations.

Major Responsibilities

Conduct face‑to‑face evaluations to confirm the presence of mental illness and determine the most appropriate setting.

Complete evaluations prior to nursing facility admissions and when significant changes indicate a need for an updated assessment.

Communicate evaluation findings to applicants, residents, and guardians in an understandable manner.

Submit determinations to facilities, agencies, or referral sources within required timelines.

Obtain necessary information and records from providers, state agencies, and participants to perform a comprehensive clinical review.

Document all relevant information in data system applications in accordance with program guidelines and regulations.

Maintain confidentiality of all business documents and correspondence per UMass Chan Medical School/ForHealth Consulting procedures and HIPAA regulations.

Participate in performance improvement initiatives and demonstrate the use of quality improvement in daily operations.

Required Qualifications

Master’s degree in social work, mental health counseling, or related human services field; or a registered nurse (RN).

Five years of direct service or case management experience with adults with psychiatric disabilities, including one year in a medical or clinical setting with knowledge of medical terminology. (Registered nurses must have a minimum 9 years of collective relevant education and work experience.)

Demonstrated knowledge of relevant social service/rehabilitation systems.

Knowledge of applicable state and federal regulations.

Ability to travel statewide.

Proficiency in Microsoft Word, Outlook, and database applications.

Preferred Qualifications

Licensed psychologist, licensed social worker, certified rehabilitation counselor, or other licensed professional in Massachusetts.

Experience with disabled or long‑term care populations.

Experience in community mental health services and one of the following areas: long‑term care, home care, rehabilitation, or disabilities.

Ability to understand and utilize resources for problem solving, effectively prioritizing and executing tasks in a high‑pressure environment.
