What we are looking for:
Mustposessa thorough knowledge of principles of effective communication, mass media, advertising, policies, education, community relations, demonstration, organization structure, social and activity services, government relations, etc., as they relate to nursing facilityoperations.
Mustpossessthe ability to plan,organizeand effectively present ideals and concepts to communitygroups/agencies.
Mustpossessthe ability to communicate effectively,orallyand inwriting.
Mustpossessthe ability to assimilate information from a variety of sources, analyzeinformationand make recommendations totheRegional Director of Sales and Marketing and theAdministrator.
Mustpossessthe ability to establish,implementandmaintaineffective marketing and public relationsprograms.
Valid Driver's License and reliable vehicle.
Ability to work flexible hours and weekends as needed.
Proficient in Microsoft Outlook, Word, Excel, and preferably CRM systems for referral tracking.
Must be at least eighteen (18) years of age.
Must be able to read and write and follow oral instructions and directions in English.
Essential Job Functions
Build and maintain strong, collaborative relationships with the Hospital Case Managers, Discharge Planners, Social Works, Preferred Provider Network Leadership, Physicians and their office staff, and Accountable Care Organizations.
Expand referral networks through consistent engagement with assisted living facilities, home health agencies, retirement communities, and senior centers.
Develop andmaintainstrong relationships with area Physicians andmaintaingood communication. Recruit new Physicians to follow residents in the facility as Attending Physicians.
Review and datacollectedon prospective residents by reviewing their medical,physicaland psychosocial needs.Coordinate with Internal AdmissionsCoordinator.
Develop and execute a Marketing Action Plan (MAP) with measurable monthly, quarterly, and annualobjectives.
Accuratelymaintain, in coordination with theInternalAdmissionCoordinator,a Daily Inquiry & Referral Tracking log.(DIRT)
Communicate and update theInternalAdmissionsCoordinatorand nursing team on prospective admissions andanticipatedadmission dates,timesand special needs.
Work in conjunction with theInternalAdmissionsCoordinatorand nursing team toassurea smooth transition of all admissions.
Conduct facility tours.
Become andmaintainan active member of community organizations.
Arrange periodical in-services with referral sources to keep them informed of the servicesthe companyprovides.
Attend communitynet-workingmeetings to maintaininformed ofchanges in the community as well as keep informed of what our competitors are doing.
Ordering and distributing marketing supplies to the community and referral sources.
Organizehealth fairs, openhousesand community lectures.
Increasing& maintainingcensus on and above budget requirements.
Follow-updaily with referrals and update records with any changesontheir medical status.
Consistently achieve and exceed census goals in alignment with budget requirements and key performance indicators (e.g.,referral conversion rates, admission timelines).
Providetimelyand professional follow-up with referral sourcesregardingacceptance decisions oradditionalinformation requests.
Crosstrainand become familiar with the job duties and responsibilities of the Call Center Coordinator and the Internal Admissions Coordinator.
Ensure all marketing and referral activities are compliant with state and federal regulations, including CMS and HIPAA guidelines.
Must be able toalways work in a constant state of alertness and in a safe manner.
Education and Experience
Master’s or bachelor’sdegree from accreditedcollege**
3-5 years'experience in thelong-termcare environment, whichincludes,SNF,ALF,HHA; and/or Hospital case management experience(with specific compliance programexperience). **
Prefer to have demonstrated management experience for a period of 3years.
Demonstrated knowledge of healthcare regulatorystandards.
Healthcare sales/marketing experiencestronglypreferred.
Working knowledge of payers for long term care environment
