invites applications for the CARE NAVIGATOR (FT/Days) position.
Overview The Care Navigator reports to the Director of Emergency Services and is part of a statewide initiative to improve access to Medication for Opioid Use Disorder (MOUD). Responsibilities include conducting initial brief assessments, introducing patients to MOUD programs and services, expediting appointments at Medication for Addiction Treatment (MAT)-capable clinics, serving as the primary coach for clients, maintaining ongoing contact with the California Bridge Program panel, and facilitating access to additional services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities to link patients from the hospital to the community.
Responsibilities
Conduct initial brief assessments.
Introduce patients to MOUD programs and services.
Expedite appointments at MAT-capable clinics.
Serve as primary coach for clients.
Maintain ongoing contact with the California Bridge Program panel.
Assist with access to financial counseling, primary care, mental health services, social services, and residential treatment facilities.
Qualifications
Associate’s degree from an accredited college or university in a health care related field required.
Bachelor's degree in substance abuse, crisis management, or counseling preferred.
Two (2) years of experience in a drug treatment environment required.
Recent in-patient drug treatment experience preferred.
Recent in-patient hospital experience preferred.
