About the Role: Join Skilled Nursing Pharmacy as a PM Staff Pharmacist and be part of a dedicated team that enhances patient care in skilled nursing facilities. Schedule: 1:00pm to 6:00pm
Responsibilities:
Provide pharmaceutical care and medication management for skilled nursing patients.
Review and verify medication orders for accuracy and appropriateness.
Consult with healthcare professionals about medication therapy and patient outcomes.
Ensure compliance with state and federal pharmacy regulations.
Participate in interdisciplinary team meetings to optimize patient care.
Educate nursing staff and patients on medication usage and safety.
Maintain accurate and up-to-date patient medication records.
Assist in training and mentoring pharmacy technicians and interns.
Requirements:
PharmD degree from an accredited pharmacy school.
Active California pharmacist license with no disciplinary actions.
Minimum of 2 years of experience in a clinical pharmacy setting.
Strong knowledge of pharmacotherapy and medication management.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Detail-oriented with strong organizational skills.
Commitment to continuous professional development and learning.
