Position Overview LTSS RN Case Manager
– Provide case management to adult members.
Location:
Austin, TX and surrounding Central Texas area.
Work Arrangement:
Hybrid – 3 days per week for member visits, work from home otherwise.
Schedule:
Monday – Friday 8 AM – 5 PM (CST).
Position Purpose:
Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex/high acuity populations with primary medical/physical health needs to promote quality and cost‑effective care. Develops personalized care plans and educates members and their families on services and benefit options.
Key Responsibilities
Evaluates the service needs of the most complex or high‑risk/high‑acuity members and recommends a plan for the best outcome
Develops and continuously assesses ongoing long‑term care plans and collaborates with the care management team to identify necessary providers, specialists, and community resources
Coordinates and manages interactions between the member/caregivers and the care provider team to ensure person‑centered care
Monitors care plans and member status for changes, collaborating with stakeholders to revise plans as needed
Monitors member status for complications and clinical symptoms, assessing potential entry into higher levels of care or waiver eligibility
Reviews member data to identify trends and improve operating performance and quality of care in accordance with state and federal regulations
Reviews referrals and intake assessments to develop appropriate care plans
Collaborates with healthcare providers to facilitate member services and determine revised care plans when needed
Collects, documents, and maintains all member information to ensure compliance with current guidelines
Provides education to members and families on disease processes, care gaps, provider instructions, and benefits
Acts as liaison and member advocate between the member/family, physician, and facilities/agencies
Educates on and coordinates community resources; provides coordination of service authorization (meals, employment, housing, transport, etc.)
May perform home or site visits as required, such as monthly assessments
Partners with leadership to improve and enhance quality of care and service delivery cost‑effectively
May precept clinical new hires by fostering core skills, coaching growth, and guiding onboarding to build readiness
May provide guidance to clinical new hires/preceptees and facilitate their transition within the MCO
May engage new hires during onboarding, ensuring completion of competency checkpoints
Engages in ongoing collaboration with People Leaders and cross‑functional teams to measure readiness
Performs other duties as assigned
Complies with all policies and standards
Education & Experience Graduate from an accredited School of Nursing or possess a Bachelor’s degree and 4–6 years of related experience.
Preferred: Bachelor’s degree in Nursing.
License / Certification
RN – Registered Nurse – State Licensure and/or Compact State Licensure required
NP – Nurse Practitioner – Current State’s Nurse Licensure required
Resource Utilization Group (RUG) certification must be obtained within 90 days of hire
