What to Expect (Job Responsibilities):
Serve as the primary point of contact for members, coordinating assessments and service plans
Facilitate the person-centered planning process and ensure appropriate service delivery
Consult with members and families regarding health care needs and benefits
Complete health care questionnaires and behavioral assessments to inform care plansDocument care coordination activities and maintain accurate records in compliance with guidelines
What is Required (Qualifications):
High School Diploma or GED equivalent required
Minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
Must have a valid driver’s license, vehicle, and verifiable insurance
Employment conditional upon successful clearance of a criminal background check and pre-employment drug screen
Intermediate proficiency level with Microsoft Office, including Outlook, Word, and Excel
How to Stand Out (Preferred Qualifications):
Experience in case management, care coordination, or home/community health management
Strong understanding of local, state, and federal healthcare laws and regulations
Ability to communicate effectively with diverse individuals and communities
Familiarity with community and state support resources
Proven organizational and time management skills
