Job Overview This position is not hybrid or available for remote work. The role is located at the Talbert Health Center in Clackamas, OR, with a schedule Monday through Friday, 8:30 a.m. to 5:00 p.m.

The Case Manager, QMHA serves as a valuable part of the treatment team for clients in a community health center setting, assisting clients to access a variety of services and supports, advocating for their needs, and working to meet their goals.

As a member of Cascadia, you will promote integrated care, trauma‑informed and person‑first approaches, and help create an inclusive environment of safety and acceptance.

Essential Responsibilities

Provide mental health case management and skills training with clients per their treatment plan.

Collaborate with the treatment team on assessing needs, developing and monitoring supports, offering consultation to mental health aides, and planning transitions.

Provide individual and group skills training, coupled with activity therapy, to meet productivity expectations.

Engage clients to participate in treatment services.

Integrate best practices (Person‑Centered Approach, Harm Reduction, Trauma‑Informed Care, Motivational Interviewing, Strength‑Based Approach, Recovery Model) into daily operations.

Create and maintain cooperative relationships with outside providers to secure benefits, advocate for clients, and connect to acute care providers.

Coordinate treatment and continuity of care with community stakeholders, legal representatives, hospital and medical staff, and other Cascadia team members.

Participate in clinical staff meetings for input, learning, and communication of program and client needs.

Supervise and support clients in developing structured activities aligned with personal goals and required conditions for community integration.

Any additional duties as assigned.

Regulatory & Compliance

Comply with all relevant laws, regulations, policies, and procedures.

Assist in crisis situations per protocols, coordinating with the treatment team.

Respect client rights, maintain professional boundaries and ethics.

Maintain accurate documentation ensuring compliance with agency policies, state and federal regulations, and billing requirements.

Understand safety regulations, participate in evacuation procedures and safety drills.

Complete annual employee training as required.

Provide guidance, support, and training to other staff aligned with the company's mission and values.

Participate in company and departmental meetings.

Qualifications Education

Required: Bachelor’s degree in a behavioral science field from an accredited institution or three years of full‑time professional experience providing direct service to individuals in a social service setting. Demonstrate competencies to become credentialed as a Qualified Mental Health Associate (QMHA) and maintain certification per OARs.

Experience

Required: Minimum of 1 year working with adults with severe and persistent mental illness.

Specialized Knowledge, Skills, and Abilities

Effective communication and active listening skills.

Interpersonal and customer service skills.

Ability to work proactively and independently within a team.

Knowledge of Microsoft Office suite.

Operational proficiency with computer hardware and software needed for job functions.

Other Requirements

Valid driver’s license with at least one year driving history; must pass DMV check and complete Cascadia Defensive Driving course.

All client‑facing positions must hold Basic Life Support (BLS) certification and be trained within the first 60 days; certification must remain current.

Working Conditions Work is performed in clinical offices, acute care settings, community settings including client residences, correctional facilities, shelters, hospitals, and other public sites. Meal periods and breaks may be interrupted. The role may extend into after‑hours for crisis response. Staff must remain alert to potential violence, disease exposure, biohazards, noise, and other environmental risks.

Tasks involve unpredictability and stress, requiring sound judgment and flexible time management.

Salary Starting Rate Range in USD:

$26.45



$27.65

per hour.
