Job Title
Care Coordinator: Sub-Acute Residential 6737
Overview Full-time, 40 hours per week. Schedule: Monday, Wednesday 12 PM-8 PM, Tuesday, Thursday, Friday 9 AM-5 PM. Program: Sub-Acute Crisis Stabilization (SACS) in Norwich.
Responsibilities
Participate as member of multi-disciplinary treatment team.
Track and coordinate client intake, discharge, services and referrals.
Assist with treatment planning and daily triage.
Educate and link individuals to self-help groups and natural support resources.
Assist with linking clients to community support systems, agencies and obtaining benefits.
Maintain documentation of contacts and communication to maintain quality services.
Work as liaison between agency and other relevant providers to coordinate care.
Assist with scheduling and providing transportation to and from appointments.
Participate in staff meetings, supervision, training and other meetings.
Provide crisis intervention and support as clinically indicated.
Additional duties as required.
Qualifications
Bachelor’s degree in human service-related field required.
Minimum of two years related experience preferred.
Valid driver’s license required.
Complete program specific training required; complete annual agency trainings.
CPR/First-Aid Certification may be required within 6 months of hire.
