About the Program Hendee Road Home is a 6 bed residential program for men with severe mental illness and who are discharging from long-term hospitalization. The program addresses the full range of daily living needs of each individual with a focus on increasing independence and self-efficacy through 24‑hour/7‑day a week staff support in the residence and community. Staff interventions teach skills around daily living, symptom management, coping skills, medication management, health & nutrition, healthy relationships, personal safety, money management, substance use education, linking to recovery supports, and providing sober activities. Staff also provide crisis intervention to reduce hospitalizations and emergency room visits.
About the Position Case Manager
Duties & Responsibilities
Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments.
Conducts program specific assessments and screens as required by program.
Provides case management, works in close collaboration with the client’s natural supports and treatment teams, connects to prosocial activities, provides crisis intervention and de‑escalation as needed.
Demonstrates competency in age‑related or specialty issues and developmental needs for each population served, and program specific target population competencies.
Demonstrates the ability to teach and support the learning of a broad range of skills including those related to emotion regulation, age‑appropriate activities and competencies, and living in a home‑like setting. Plans and actively participates in activities to support and increase skills and competencies.
Documents all services and contacts with clients, collateral contacts, and appointments in compliance with agency standards.
Transports clients as required by program.
Participates in staff meetings, supervision, training, and other meetings as assigned.
Additional duties as required.
Qualifications Education:
Bachelor’s degree in human services preferred, or education plus equivalent work experience required.
Experience:
At least two (2) years’ experience in a mental health setting preferred.
Licensure/Certification/Registration:
Valid driver’s license.
