PURPOSE: The OnTrackNY program is part of the Community Behavioral Health Division located at our Crossroads Clinic. The program serves adolescents and young adults who have experienced early psychosis. The model uses a multidisciplinary team approach. Professionals use evidence‑based interventions and recovery‑oriented models to enable participants to reach goals and reduce disability. The program provides emotional and practical therapy and support to help participants and their families in their recovery journeys. The multidisciplinary team includes the Team Leader, Primary Clinicians, Outreach and Recruitment Coordinator, Supported Education and Employment Specialist, Peer Specialist, Psychiatrist, and Registered Nurse. Services are either provided at the clinic site, in the home, or other off‑site locations that are convenient for the participants and their families. 24‑hour on‑call services are included. Intensive education will be offered to all members of the OnTrackNY Team regarding the evidence‑based model and their roles for members to adhere to fidelity standards and successfully support program participants.

POSITION OVERVIEW The Team Director is a member of the program’s leadership team and is primarily responsible for clinical services and administrative operations of a distinct program/service within a larger, existing community program. The Team Director is responsible for clinical supervision and professional development of staff, quality service delivery, and building and maintaining collaborative relationships with funders and partners. Additionally, this position is responsible for ensuring clients receive timely access to quality clinical services; overseeing administrative functions to support program operations; and ensuring program compliance with federal, state, and City regulatory bodies and funding entities.

RESPONSIBILITIES

Clinical Services:

Identify gaps in clinical services at the program level and develop potential solutions to fill the gap.

Lead the implementation of evidence‑based treatment or have direct oversight and responsibility for innovative models of care, in partnership with JB training department and/or external agencies.

Ensure clients have timely access to care by ensuring admissions and discharges are completed in a timely manner, assigning cases in a timely manner and ensuring clinicians have adequate case load sizes to minimize clients’ wait for services.

Monitor show rates and/or census and appointment availability.

Outreach to the community representing the array of services offered and discussing referral process.

Build collaborative relationships and working partnerships with community members and other organizations.

Provide direct services to clients, as needed, including intakes and crisis interventions.

Supervision of crisis intervention services.

Other clinical duties as assigned.

Implementation of quality improvement initiatives.

Participate in quality assurance activities such as chart reviews and utilization management.

Provide individual and group supervision to program staff.

Integrate recovery, rehabilitation, treatment and vocational components of the program.

Monitor and follow up on program incidents.

Personnel:

Participating in hiring and orienting of new staff.

Timely completion of performance appraisals and appropriately addressing performance concerns.

Conducting or participating in staff training and development.

Monitor staff attendance and vacation scheduling to ensure program coverage.

Supervision of staff, as appropriate to licensure.

Plan and participate in other program activities.

Administrative Functions and Program Operations:

Facilitate team meetings, case conferences, and other meetings as assigned.

Ensure program and staff are in compliance with regulations and standards of care that govern the program.

May be responsible for completing budgets and/or providing significant input into the budgeting process.

Utilize a variety of electronic databases to monitor billing and address any fiscal or billing issues that arise.

Obtain, track, and analyze data specific to the program requirements and for reporting purposes.

Participate in external program meetings and events required by funding and/or governing bodies.

Identify and develop systems to improve program operations.

May serve as back up to the Director or Program Supervisor I, assuming responsibility for operations in their absence.

Other administrative duties as assigned.

EDUCATIONAL / TRAINING REQUIRED

Master’s degree in human services and New York State clinical licensure in social work or mental health counseling (LCSW, LMSW, or LMHC), consistent with regulatory and/or contract requirements.

At least 3 years prior experience in a supervisory/leadership role in a behavioral health program.

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE

Training and experience working with adolescents and young adults with serious mental illness: desirable.

Experience providing employment and educational services: preferred.

Solid clinical skills and knowledge of evidence‑based practices.

Excellent organizational skills, teamwork, and communication skills.

COMPUTER SKILLS REQUIRED

Use of an Electronic Health Record (EHR).

Use of Outlook and related Microsoft Office Applications.

VISUAL AND MANUAL DEXTERITY

Able to read/input data and documents, including spreadsheets, reports and Electronic Health Records in printed form and on computer screens.

Able to input data into the Electronic Health Record.

Limited applications of manual dexterity and hand‑eye coordination.

WORK ENVIRONMENT / PHYSICAL EFFORT

Noise level is consistent with levels usually present in an office, education, rehabilitation or health related environment when in office.
