Schedule Full‑time (37.5 hours per week) Monday‑Friday 9 a.m. – 5 p.m.
Pay Starting hourly rate: $19.70.
Description The Residential Coordinator provides case‑management services by planning and implementing individualized service plans for adults with mental illness and secondary substance‑abuse problems, who reside in MHA’s supportive training apartment program. Services are client‑centered and aim to enhance functioning levels and quality of life. The coordinator assists individuals who live independently in the community and schedules visits with them several times per week.
Responsibilities
Write individual service plans, service plan reviews, monthly progress notes, and all admission and discharge paperwork.
Maintain daily progress notes, manage goal contacts, and supervise medication administration.
Ensure consumers receive proper medical care and link them with vocational and educational opportunities in the community.
Qualifications
High School Diploma and passion for working with adults who have significant mental health and substance‑abuse behaviors.
Excellent writing, computer, organizational, and interpersonal skills; ability to prioritize tasks and work independently.
Clean, valid NYS driver’s license with at least 3 years of driving experience.
Physical ability to lift up to 25 lbs.
