Circle of Life is growing and we’re looking for a
Float Designated Coordinator
to join our Bemidji and Cass Lake teams. We are a leading home care agency serving six states, providing services for seniors, children, and people with disabilities. Our mission is to honor those we serve and to help clients stay independent in their own homes and communities.
Pay Range $25.00/hr - $33.00/hr
Responsibilities
245D compliance and paperwork
Train, schedule, and provide ongoing support and evaluation of direct care staff
Manage services
Travel weekly to individuals’ homes and various branch offices across the assigned region
Evaluate the effectiveness of service delivery, methodologies, and progress on the individual’s outcomes
Facilitate the achievement of the outcomes as specified in section 245D.07
Communicate with individuals, guardians, case managers, behavior analysts, and other team members to decide on services needed to support everyone most effectively
Maintain and summarize data for reports
Coordinate appointments for individuals as requested
Assist Director with quality improvement projects
Problem solve when emergencies arise
Provide work direction, guidance, training, and feedback to staff to ensure the highest standards of service delivery
Qualifications
(1) A baccalaureate degree in a field related to human services and one year of full‑time direct care experience with people with disabilities or age 65+
(2) An associate degree in a related field and two years of full‑time direct care experience with people with disabilities or age 65+
(3) A diploma from an accredited postsecondary institution in a related field and three years of full‑time direct care experience with people with disabilities or age 65+
(4) Minimum of 50 hours of education and training related to human services and disabilities
(5) Four years of full‑time direct care experience with people with disabilities or age 65+ under a staff person who meets the qualifications in clauses (1) to (3)
The license holder must verify and document the competence of one of the above
