Responsible for implementation of daily activities of individuals in his/her charge on and off the dayhab site.

Primary Duties and Responsibilities

Implement habilitation plans for participants by reviewing records, observing and providing feedback to improve participants’ skills. Responsible for documentation of site and community based activities.

Assist and support participants in developing and maintaining their independent functioning. This may include all areas of personal hygiene (i.e. toileting, handwashing, showering, tooth brushing, hair care, nail care, dressing and eating).

Utilize all information in the Protective Oversights to provide needed support to the individuals.

Implement appropriate community based activities for participants.

Train and support participants as they perform volunteer activities in the community to the satisfaction of staff at the community site.

Ensure protection of participants as they perform volunteer activities in the community to the satisfaction of staff at the community site.

Responsible for overseeing the safe arrival and departure of participants to and from the Day Habilitation site.

Assist in maintaining an orderly work environment. Assist in keeping equipment and supplies in good condition.

May act as an advocate to address concerns on behalf of participants’ rights.

Transport participants to and from community outings. Ensure safety of participants during community activity.

Reports changes in participants’ physical condition to the Day Habilitation nurses and supervisor in order to ensure that medical needs are met in a timely fashion.

Performs additional duties as requested by supervisor.

Ensure that work‑related safety procedures are in place and are being adhered to by staff.

Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.

Qualifications

H.S. or equivalent preferred.

Minimum one year working with the developmentally disabled population preferred.

Valid and qualified NYS driver’s license required.

Good oral and written communication skills.

CPR/First Aid and defensive driving training preferred.
